Schedule/Hours:

Day shifts only No weekends or holiday shifts required

Key Responsibilities:

Complete Start of Care (SOC) visits, particularly orthopedic patients when nursing services are not ordered Deliver patient-centered physical therapy in home settings, averaging five visits per day across Tehama County (mileage generally within 40 miles daily) Utilize the HomeCare HomeBase (HCHB) EMR on a provided tablet to document patient care Work collaboratively with intake RNs, management, and nurse aides to ensure continuity of quality care

Qualifications:

Active PT license BLS certification required At least some home health experience (minimum requirement regardless of total years in practice) Comfort and proficiency with diverse diagnoses, especially orthopedic and geriatric, is highly valued Ability to travel for work within the local region
