Active California Physical Therapist license required to start the assignment. BLS Certification required from AHA or ARC. Home health experience required. HCHB (Homecare HomeBase) experience required.

Productivity familiarity:

ability to meet productivity expectations (example: approximately 6 points per day via combinations of follow-up and Start of Care visits). Ability to provide services in the Woodland Hills and Encino territory (91302 and 91316) and manage travel within that territory. Reliable transportation and ability to perform home visits in a community setting.
