Physical Therapist (PT) Lead with Clinical Excellence at Diablo Valley Post Acute Are you a Physical Therapist who thrives in a collaborative, "fun-first" clinical environment? Diablo Valley Post Acute in Concord is seeking a dedicated PT to join our premier rehabilitation team. As a leading provider of short-term recovery and long-term care, we operate a dynamic, 7-day-a-week therapy program. Here, we prioritize high-quality patient outcomes and a supportive culture where professional respect is part of the daily routine.
The Opportunity Location:
Concord, CA (Contra Costa County)
Compensation:
Starting at $60.00/hour (DOE)
Availability:
Full-Time, Part-Time, and PRN Flexible Shifts:
5-day schedules (Tue-Sat or Sun-Thu) — Skip the Monday rush!

Your Impact & Responsibilities Clinical Leadership:
Perform evaluations and develop physician-approved treatment plans tailored to each resident's unique recovery journey.
Team Supervision:
Provide expert supervision of PTAs in accordance with the California State Practice Act.
Interdisciplinary Teamwork:
Partner with medical and nursing staff to communicate goals, progress, and discharge strategies.
Resident Advocacy:
Lead discharge planning and recommend DME to ensure a safe and successful transition home.
Education:
