1 Description American Traveler is seeking a Physical Therapist with Home Health experience for a 13-week travel contract serving a geriatric patient population across a defined territory in the Far East Bay Area of California. Details Home Health setting serving a geriatric patient population Territory covers the Far East region, spanning Discovery Bay to Pittsburg Average patient load of 5-7 patients per day Day shift schedule, Monday-Friday, 8:00 AM-5:00 PM (5x8-hour shifts) No on-call or weekend requirements

EMR:

HCHB (experience required); OASIS experience preferred iPad provided for documentation; mileage reimbursed at the IRS standard rate Requirements Minimum 1 year of recent Home Health PT experience required Current BLS certification required Active PT license required if applicable

experience required Must be willing to travel within a territory of up to 60 miles from the hub/office Must be able to meet productivity standards Additional Information Travelers are expected to be flexible with territory assignments OASIS experience is preferred but not required First-time travelers are welcome to apply Local candidates are accepted and eligible for the travel rate Scrubs required in any color; jeans are not permitted 2 professional supervisor references from within the past 2 years required for consideration Full 7-year work history with all gaps explained required for consideration All time-off requests must be submitted at the time of initial application
