Schedule/Hours:

Day shift, no nights No weekends or holidays No on-call requirements

Qualifications:

Licensed Physical Therapist (PT) with Home Health experience (all experience levels considered) Active BLS certification Previous experience using EMR, preferably HCHB (HomeCare HomeBase) Strong interpersonal and communication skills Ability to conduct about 5 home visits per day, totaling approximately 40 driving miles (with potential for reduction as the team expands)

Responsibilities:

Perform Start of Care (SOC) visits, with particular focus on orthopedic patients Deliver evidence-based therapy to patients with varied and complex medical conditions Collaborate with Intake RN, office managers, and nurse aides to optimize patient outcomes Complete comprehensive, timely electronic documentation using provided tablet Participate in routing and redistributing patient visits as the caseload grows
