Key Responsibilities:

Evaluate and assess patients' physical conditions and develop tailored treatment plans. Implement effective physical therapy interventions to enhance mobility, strength, and functional ability. Collaborate with interdisciplinary teams to ensure comprehensive and holistic patient care. Maintain accurate and timely documentation of patient progress and treatment plans.

Qualifications:

Valid Physical Therapy license in [State] Proven experience in a clinical or skilled nursing facility setting is preferred.
