Travel PT Home Health jobs in Alameda and Contra Costa, CA are available for an ASAP start on a 13-week contract, working Monday through Friday 8-hour days full-time. You must have home health experience and a current California PT license. The caseload is 5-6 visits per day, and you will document using Kinzer EMR. Your responsibilities include providing skilled physical therapy in patient homes, developing individualized care plans, and collaborating with the healthcare team to support mobility and independence.
