Per-visit rates from $87 to $200 What You'll Do Deliver high-quality physical therapy services in a home setting Conduct assessments and develop individualized care plans Educate patients and caregivers on exercises and mobility techniques Collaborate with interdisciplinary teams to ensure holistic care Use advanced, user-friendly technology to document and track progress What You Bring Degree from an APTA-accredited physical therapy program Active state license to practice as a Physical Therapist At least 1 year of experience or completion of a 6-8 week preceptorship Home health experience and knowledge of Medicare Conditions of Participation preferred Strong organizational, communication, and tech skills Valid driver's license and reliable transportation Benefits & Perks Medical, Dental, Vision Insurance
