Physical Therapist (PT)

  • 65/HR BOE | Skilled Nursing Facility Are you a compassionate and skilled Physical Therapist looking to make a meaningful impact in the lives of others?
Join us at Oakwood Gardens Care Center, a skilled nursing and rehabilitation facility committed to helping our residents regain independence and achieve their highest level of function.
What You'll Do:
Evaluate residents to determine therapy needs and create individualized treatment plans Provide direct physical therapy treatments in accordance with physician orders and resident goals Document evaluations, progress notes, and treatment records accurately and timely Collaborate with an interdisciplinary team to ensure comprehensive resident care Supervise and support Physical Therapy Assistants (PTAs) Participate in discharge planning and caregiver education to ensure continuity of care Maintain compliance with state, federal, and facility regulations
What We're Looking For:
Licensed Physical Therapist (PT) in the State of California Strong clinical skills and dedication to resident-centered care Ability to work independently as well as part of a team Excellent communication and organizational skills Skilled nursing or post-acute rehab experience preferred, but not required
