Flexible Schedule Location:
Central/South Orange County We're looking for passionate Physical Therapists to join our growing home health team! What You'll Do:
Deliver 1:1 skilled physical therapy care to patients in their homes—no clinic walls, just real connection. Perform comprehensive physical therapy evaluations and establish individualized, evidence-based care plans. Provide hands-on therapeutic interventions to improve strength, mobility, balance, endurance, and functional independence. Monitor and document patient progress, outcomes, and response to treatment, adjusting the plan of care as clinically indicated. Educate patients and caregivers on home exercise programs, safety strategies, and mobility techniques to promote carryover and prevent injury. Collaborate closely with a multidisciplinary team including nurses, occupational therapists, speech therapists, and physicians. Maintain a safe, respectful, and professional home care environment that supports patient dignity and comfort.
What You Bring:
Licensed PT CA (required) Strong communication skills and a compassionate approach to care Home health experience is a plus, but not required Familiarity with orthopedic and/or geriatric populations a bonus Ability to work independently—and still be a team player Reliable transportation for home visits
