Physical Therapy Field Home Health Care

Position Summary Provides skilled physical therapy services to patients in their home setting, working under a physician's plan of care to restore function, improve mobility, and support patient independence. Key Responsibilities Conduct initial evaluations and develop individualized plans of care Deliver skilled PT interventions including therapeutic exercise, gait training, balance/fall prevention, and manual therapy Educate patients and caregivers on home exercise programs and safety Coordinate with the interdisciplinary team (RN, OT, SLP, MSW) and attending physician Document all visits, progress notes, and discharges in the EMR in a timely manner Participate in OASIS assessments as applicable Maintain compliance with Medicare/Medicaid conditions of participation and agency policies Qualifications Graduate of an accredited physical therapy program (DPT preferred) Current state PT licensure in good standing CPR certification required Minimum 1 year of clinical experience; home health experience preferred Valid driver's license and reliable transportation Strong communication and time management skills
