A Day in the Life (Your Responsibilities) Conduct comprehensive physical therapy evaluations of residents to assess mobility, strength, balance, endurance, and functional abilities. Develop and implement individualized treatment plans based on physician orders and patient needs. Provide therapeutic exercises, gait training, transfer training, balance activities, and mobility interventions. Educate residents, families, and caregivers on safety techniques, exercise programs, and adaptive equipment. Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes. Maintain accurate and timely documentation in accordance with facility, state, and federal regulations. Collaborate with interdisciplinary team members, including Occupational Therapists, Speech Therapists, Nursing Staff, Social Workers, and Physicians. Participate in care planning meetings, quality improvement initiatives, and continuing education activities. Ensure compliance with all Skilled Nursing Facility policies and rehabilitation standards. What It Takes to Succeed (Qualifications)License Current and unrestricted Physical Therapist (PT) license in the State of California (Required). Education Doctor of Physical Therapy (DPT) or degree from an accredited Physical Therapy program. Experience Skilled Nursing Facility (SNF), rehabilitation, long-term care, or geriatric experience preferred. New graduates with strong clinical rotations are encouraged to apply. Certification Current Basic Life Support (BLS) certification. Skills Strong clinical assessment and treatment planning skills. Excellent communication and interpersonal abilities.
