In this vital role, you will assess patient needs, develop personalized care plans, and deliver targeted treatments to promote recovery, mobility, and independence. Your expertise will support patients across diverse populations, including pediatrics, geriatrics, and those recovering from acute or chronic conditions. This position offers an energizing opportunity to bring compassionate care directly to patients where they feel most comfortable—at home. Duties Conduct comprehensive assessments of patients' physical function, mobility, and pain levels in home and community environments Develop and implement individualized care plans focusing on physiotherapy techniques such as trigger point therapy, orthopedics, and rehabilitation strategies Deliver hands-on treatments including therapeutic exercises, manual therapy, and modalities tailored to each patient's condition Coordinate discharge planning with healthcare teams to ensure seamless transitions of care and optimal recovery outcomes Educate patients and caregivers on injury prevention, exercise routines, and health management strategies Maintain detailed documentation of patient progress using electronic medical record (EMR) systems in compliance with healthcare regulations Collaborate with multidisciplinary teams across inpatient, outpatient, hospital, clinic, and home health settings to optimize patient care Skills Strong knowledge of physiology, anatomy, and medical terminology essential for accurate assessment and treatment planning Proven experience in home & community care environments with a focus on patient-centered care delivery Expertise in discharge planning, care plan development, and coordination across various healthcare settings Skilled in treating diverse populations including pediatrics, geriatrics, orthopedics, and patients with acute or chronic conditions Familiarity with EMR systems for efficient documentation and communication within healthcare teams Knowledge of outpatient therapy practices such as trigger point therapy and orthopedic rehabilitation techniques Excellent understanding of inpatient hospital protocols and home health procedures to ensure safe and effective treatment delivery This energetic role empowers you to bring your physiotherapy expertise into the community—helping patients regain their strength and independence while fostering a positive impact on their quality of life.
