This is a permanent, full-time opportunity with an ASAP start — ideal for a clinician who thrives in a collaborative SNF environment.

ASAP About the Role As a Physical Therapist, you will evaluate and treat a geriatric patient population recovering from surgery, illness, or injury in a post-acute setting. You will manage an average caseload of 10 patients per day , with a productivity expectation of 87-90% . The facility uses PCC and Net Health as their EMR systems. You will be part of a team of 5 therapists in a flexible, cooperative environment where collaboration and patient-centered care are at the heart of everything they do. Key Responsibilities Evaluate and develop individualized physical therapy treatment plans for geriatric patients Deliver skilled therapeutic interventions including therapeutic exercise, functional mobility training, and manual techniques Maintain 87-90% productivity while delivering high-quality patient care Accurately document patient progress and care plans in PCC and Net Health Collaborate with an interdisciplinary team including OTs, SLPs, nursing, and physicians Supervise Physical Therapy Assistants (PTAs) as applicable Participate in discharge planning and care coordination Requirements Current California PT license (or eligibility to obtain) Graduate of an accredited Physical Therapy program (DPT preferred) Minimum 1-2 years of SNF or post-acute experience required Proficiency or willingness to learn PCC and Net Health EMR Strong compliance, professionalism, and communication skills Team-oriented, flexible, and patient-focused work style Why Apply Through Us?
