$58-$62 Per Hour We are actively seeking a compassionate and motivated Physical Therapist (PT) to join an Outpatient Clinic in Milpitas, CA through Therapy HCR. This is an excellent opportunity to work in a dynamic outpatient setting helping patients improve mobility, reduce pain, restore function, and achieve meaningful recovery outcomes. Key Responsibilities Evaluate and assess patients to develop individualized treatment plans. Deliver evidence-based physical therapy interventions to improve strength, mobility, balance, and overall function. Monitor patient progress and modify treatment plans as needed to optimize outcomes. Educate patients and caregivers on rehabilitation goals, home exercise programs, and injury prevention. Maintain timely and accurate documentation in compliance with clinic and regulatory standards. Collaborate with physicians and interdisciplinary care teams to ensure coordinated patient care. Participate in discharge planning and continuity of care initiatives. Promote a positive and patient-centered therapy experience. Qualifications Active California Physical Therapist (PT) License. Graduate of an accredited Physical Therapy program. Outpatient experience preferred, but not required. Strong clinical reasoning, communication, and documentation skills. Ability to work effectively in a collaborative and patient-focused environment.
