Job Description

Job description: Under the direction of the Administrator and in coordination with the Program Director Physical therapist consultant provides/supervises the treatment of Adult Day Health Care participants. provide consultation to the administrator and is a member of the multidisciplinary team. The physical therapist will ensure compliance with all
ADCHC/ CBAS
Laws, regulations, CBAS certifications standards, and the standards of participation.
Duties:
1)Evaluate all participants' physical conditions and functional status. Establishes a realistic goal and sets up a treatment plan to administer physical therapy techniques. Consults with participant family and caregiver to determine wants, needs, and goals for day center attendance and physical therapy interventions. Completes signed, and dated quarterly progress notes, and six-month reassessments on all participants receiving physical therapy. Revaluates patient for any changes in health or functional status. 2) Provides physical therapy treatment. Instructs and demonstrates to family and others the physical therapy and rehabilitation procedures indicated. 3)Provides general instructions in principles of physical therapy to staff members as needed. 4) In coordination with the center R.N. supervises maintenance aids by instruction, observation, and open discussion. 5) Records on participant's chart as daily. Record initial and final evaluations, make progress notes, and schedules participants for further treatment. 6) Works in close cooperation with staff and participants as a member of the multidisciplinary team. 7) Provides a maintenance-training program to center staff and periodic evaluation of those receiving training. 8) Meet the administrative requirements of the position such as attending meetings and keeping statistical records. 9) Participates in the discharge plan and initials the discharge summary Conditions of contract: Must have current CPR and First Aid certifications. Must have had a TB clearance within the last 12 months. Must work the hours necessary to meet the needs of the participants.
Qualifications:
1)Knowledge of the principles and procedures relating to physical therapy and how they apply specifically to frail elderly. 2) Experience with frail elderly or chronically ill persons and as a member of a multidisciplinary team. 3) Skills in assessing and reassessing the functional capabilities of the frail elderly; in identifying and establishing individual treatment programs for physical therapy; administering the prescribed physical therapy program; in communicating the principles of physical therapy to other staff members and in keeping accurate, ongoing records of individual therapy programs. 4) Licensed by the Physical Therapy Board of California.
