We are privately owned, CHAP Accredited Home Health Agency based in Campbell, California and have just launched our new East Bay office. We provide care all throughout the greater Bay Area. We have an immediate need for per diem or part time to full time Physical Therapist in Napa County. This position will quickly grow to full time. Our founders include 150+ years of highly experienced nurses, healthcare administrators and other professionals in the post-acute industry dedicated to the highest quality clinical outcomes and patient satisfaction.

Compensation:

Up to $85 per hour DOE, Sign On

BONUS+ 100

% paid medical for full time candidates. Responsibilities Conduct thorough assessments of patients' physical abilities and limitations. Develop individualized treatment plans based on patient evaluations and goals. Implement therapeutic exercises and modalities to promote recovery and rehabilitation. Provide education to patients and their families regarding treatment procedures and home exercises. Collaborate with other healthcare professionals to ensure comprehensive patient care. Monitor patient progress and adjust treatment plans as necessary. Participate in discharge planning to facilitate smooth transitions from inpatient to outpatient care or home community care. Maintain accurate documentation of patient progress and treatment outcomes. Experience A degree in Physical Therapy from an accredited institution is required. 1+ year experience in a home health setting Knowledge of anatomy, physiology, and rehabilitation techniques is essential. Experience with acute care settings, pediatrics, or discharge planning is a plus. Strong interpersonal skills with the ability to communicate effectively with patients, families, and team members. A valid state license to practice physical therapy is required.
