Licensed Physical Therapist Wellness Consultant (PRN) Divinity Wellness Ontario, CA Job Details Contract $65 - $85 an hour 18 hours ago Benefits Flexible schedule Qualifications Assisting with ADLs Physical Therapy License Motor function examinations Group home experience Full Job Description Looking for a motivated, engaged , compassionate, licensed Physical Therapist for contract work within the Ontario and Chino California areas. This independent contract position works directly with individuals in their congregate living homes. As the lead physical therapist, the therapist will provide Wellness consultations and care oversight for residents within a congregate living home, with a focus on providing Wellness activities focused on functional status, activities of daily living, independent living skills, fine motor coordination, home safety recommendations, adaptive equipment training, strengthening and maintaining general strength and mobility. The Physical therapist wellness consultant will develop the treatment plan and collaborate with the care team to ensure that the activity plan is followed, performing activities aimed at improving and enhancing the patient's wellbeing, and monitoring the patient's progress. The successful candidate will also collaborate with supported living center staff ( nurse, direct support professionals, house administrator) to enhance the physical, mental, and social well-being for patients. This position allows for flexible hours, efficient documentation, setting your own schedule, stability of hours, and working as a part of a caring team to provide compassionate care to individuals that will benefit from your skills !
