Key Responsibilities:

Conduct patient evaluations and develop individualized therapy plans Implement treatment plans and monitor patient progress Educate patients and caregivers on exercises and safety practices Maintain accurate, timely, and compliant documentation Collaborate with interdisciplinary teams to support patient outcomes Participate in ongoing training and professional development Support patients and families throughout the rehabilitation process

Qualifications:

Doctor of Physical Therapy (DPT) with active California license Minimum of 1 year of experience, preferably in home health Strong knowledge of rehabilitation techniques and care planning Experience with electronic health record systems CPR certification preferred Apply Today!
