We are urgently seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will possess a strong foundation in physiotherapy, with the ability to deliver comprehensive patient care across diverse settings. This role offers an exciting opportunity to make a meaningful impact on patients' recovery journeys by applying expert knowledge in anatomy, physiology, and therapeutic techniques. The successful applicant will be responsible for developing individualized care plans, coordinating discharge planning, and providing exceptional therapy services in inpatient, outpatient, hospital, home health, and community care environments. Duties Assess patients' physical conditions through detailed evaluations, utilizing expertise in anatomy and physiology to inform treatment strategies. Develop and implement personalized care plans tailored to patient needs, including those in pediatrics, geriatrics, orthopedics, and other specialties. Deliver physiotherapy interventions such as trigger point therapy, mobility exercises, and functional training to promote recovery and improve quality of life. Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless transitions between care settings. Maintain accurate documentation of patient progress within electronic medical record (EMR) systems while adhering to confidentiality standards. Educate patients and their families on treatment routines, home exercises, and community resources to support ongoing recovery. Monitor patient responses to therapy, adjusting treatment plans as necessary based on clinical observations and progress. Ensure compliance with hospital protocols, safety standards, and best practices in inpatient, outpatient, and home health environments. Qualifications Valid licensure or certification as a licensed Physical Therapist with relevant regulatory bodies. Proven experience in acute care hospital settings with a focus on inpatient rehabilitation and medical terminology proficiency. Strong knowledge of anatomy, physiology, orthopedics, geriatrics, pediatrics, and community-based care practices. Experience working with EMR systems for documentation and patient management. Demonstrated ability to develop effective care plans incorporating discharge planning and patient education. Familiarity with physiotherapy techniques such as trigger point therapy and orthopedic rehabilitation methods. Excellent communication skills for engaging with patients of diverse backgrounds and collaborating within multidisciplinary teams. Home & community care experience is highly desirable to support outpatient and home health services.
