Responsibilities Conduct comprehensive patient evaluations to determine physical therapy needs across various populations including pediatrics, geriatrics, orthopedics, and post-surgical cases. Develop and execute tailored care plans utilizing evidence-based physiotherapy techniques such as trigger point therapy and manual therapy to optimize recovery. Assist patients with mobility training, pain management, and functional independence through active treatment sessions. Coordinate discharge planning and collaborate with multidisciplinary teams to ensure seamless transitions between inpatient, outpatient, home health, and community care services. Maintain accurate documentation of patient progress using Electronic Medical Record (EMR) systems while adhering to medical terminology standards. Educate patients and their families on injury prevention, physiotherapy exercises, and home care strategies to support long-term health goals. Stay current with advancements in physiology knowledge, orthopedics, and care practices to continually enhance treatment quality. Qualifications Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction. Proven experience in inpatient hospital settings, outpatient clinics, or home health environments demonstrating strong patient care skills. Familiarity with EMR systems for documentation and treatment planning purposes. Knowledge of anatomy and physiology principles essential for effective therapy interventions. Experience with discharge planning, acute care procedures, and developing comprehensive care plans. Ability to work effectively within multidisciplinary teams across diverse healthcare settings including pediatrics and geriatrics.
