Day shifts No weekends or holiday shifts required

Qualifications:

Current Physical Therapy license and BLS certification Home Health experience required (travelers with less than 2 years' overall experience will be considered with Home Health expertise) Competency with HomeCare HomeBase EMR preferred Ability to work independently with a daily average of 5 patient visits Valid driver's license (mileage reimbursed)

Role Responsibilities:

Conduct start-of-care assessments, especially for orthopedic patients when skilled nursing isn't ordered Manage a flexible caseload within a growing census, supported by an Intake RN, Manager, and nurse aides Provide compassionate, evidence-based PT for patients in their homes across most of Tehama County Complete documentation using a provided tablet Collaborate with leadership on efficient patient load distributions as staff expands
