The ideal candidate will have a strong background in various settings, including home health, outpatient clinics, and acute care environments. This role involves assessing patient needs, developing personalized treatment plans, and providing high-quality patient care to enhance mobility and improve overall physical function. Responsibilities Conduct thorough evaluations of patients to determine their physical therapy needs and establish appropriate treatment goals. Develop and implement individualized treatment plans based on patient assessments, incorporating techniques from orthopedics, geriatrics, and other relevant specialties. Provide hands-on therapy sessions to patients in various settings, including home health and outpatient clinics. Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes. Educate patients and their families about exercises, techniques, and lifestyle modifications to support recovery. Collaborate with other healthcare professionals to coordinate comprehensive care for patients. Participate in discharge planning to ensure a smooth transition for patients leaving therapy services. Skills Strong knowledge of physiology and physical therapy principles. Experience in home health, outpatient settings, geriatrics, acute care, and clinic environments is highly desirable. Proficient in discharge planning processes to facilitate effective patient transitions. Excellent communication skills for interacting with patients, families, and interdisciplinary teams. Ability to develop rapport with patients while demonstrating empathy and understanding of their challenges. Strong organizational skills to manage patient caseloads effectively.
