In this role, you will help residents improve mobility, restore function, and maximize their independence through individualized, evidence-based treatment plans. Whether you're an experienced SNF therapist or new to the setting, mentorship is available to help you thrive. What You'll Do Evaluate residents and develop individualized treatment plans Provide skilled physical therapy services to improve mobility, strength, balance, and functional independence Collaborate with physicians, nursing staff, and the interdisciplinary care team to achieve optimal patient outcomes Complete accurate and timely documentation in Net Health according to facility standards Float to a nearby sister facility within walking distance as needed Qualifications Active California Physical Therapist (PT) license required Skilled nursing facility (SNF) experience preferred, but not required Mentorship available for therapists with 0-1 year of experience Strong documentation, organization, and time-management skills Flexible, team-oriented, and patient-focused approach to care Schedule Preferred availability: Sunday-Thursday, or Tuesday-Saturday Why Join Us?
