13 Weeks Position Overview We are seeking a skilled and compassionate Acute Care Physical Therapist (PT) to join a multidisciplinary rehabilitation team in Apple Valley, California. This role focuses on providing high-quality physical therapy services within an acute care hospital environment, supporting patients through evaluation, treatment planning, mobility training, discharge planning, and rehabilitation recovery. The ideal candidate will have strong acute care experience and be comfortable working with a diverse patient population including orthopedic, cardiac, neurological, ICU, and medical-surgical patients. Key Responsibilities Perform comprehensive physical therapy evaluations and reassessments Develop and implement individualized treatment plans and home exercise programs Assist patients with mobility training, gait training, transfers, and strengthening exercises Provide patient and family education regarding rehabilitation goals and recovery plans Collaborate with physicians, nurses, and interdisciplinary healthcare teams Monitor patient progress and adjust treatment plans as needed Support discharge planning and continuity of care Document patient treatments and progress accurately within the EMR system Ensure patient safety and adherence to facility protocols during therapy sessions Requirements Active California Physical Therapist (PT) License required Current BLS Certification required Minimum 1 year of Acute Care PT experience preferred First-time travelers accepted Strong experience with: Acute Care Rehabilitation Mobility & Transfer Training Orthopedic Rehabilitation ICU & Medical-Surgical Patient Care Geriatric Patient Care Patient & Family Education Discharge Planning Strength & Functional Assessments Clinical Experience Preferred Candidates with experience treating the following conditions are strongly preferred:

COPD CHF CABG

Recovery Myocardial Infarction (MI) Post-Stroke Rehabilitation Hip Fractures/Injuries Knee Injuries/Tendinopathy Total Hip Replacement Total Knee Replacement Equipment & Skills Experience using and supporting: Gait Belts Walkers & Crutches Hospital Beds Supplemental Oxygen Assistive Devices Body Mechanics & Safe Transfer Techniques Vital Signs Monitoring
