In this vital role, you will assess, develop, and implement customized physiotherapy treatment plans to promote recovery, improve mobility, and enhance quality of life for our diverse patient population. Your expertise will support patients across various settings, including inpatient, outpatient, home health, and community care environments, ensuring seamless transitions and optimal outcomes. Duties Conduct comprehensive evaluations of patients' physical abilities, medical history, and physiology knowledge to formulate effective care plans. Develop and implement individualized treatment strategies that incorporate physiotherapy techniques such as trigger point therapy, orthopedics, geriatrics, pediatrics, and geriatrics. Assist patients with mobility, strength, balance, and pain management through therapeutic exercises and modalities tailored to their needs. Collaborate with interdisciplinary teams to coordinate discharge planning and ensure smooth transitions from hospital or inpatient settings to home or community-based care. Utilize electronic medical record (EMR) systems to document assessments, treatment progress, and discharge instructions accurately. Educate patients and their families on injury prevention, post-treatment exercises, and home care routines to promote long-term health benefits. Stay current with medical terminology updates and anatomy knowledge to deliver precise and effective physiotherapy interventions. Requirements Valid licensure or certification as a licensed Physical Therapist in the respective region or country. Proven experience in Skilled Nursing Facility. Strong understanding of physiology knowledge relevant to orthopedics, geriatrics and general patient care. Experience with discharge planning processes and developing comprehensive care plans for diverse populations. Familiarity with EMR systems for documentation and patient management. Knowledge of medical terminology and anatomy to accurately assess and treat various conditions. Excellent communication skills to collaborate effectively with healthcare teams and educate patients about their treatment plans.
