Job Description

Key Responsibilities 1. Patient Assessment and Treatment
  • Conduct initial and ongoing assessments to identify patient needs and limitations
  • Develop individualized treatment plans with goals based on assessment findings
  • Administer and modify therapeutic interventions, including exercises, manual therapy, and functional training 2. Patient and Family Education
  • Provide guidance on safety, injury prevention, and home exercises to improve mobility and strength
  • Educate caregivers and family members on assisting with daily exercises and therapeutic activities
  • Address patient and family questions, offering encouragement and support 3. Documentation and Compliance
  • Document each visit in compliance with state and federal guidelines, capturing progress and any changes
  • Maintain accurate, up-to-date records, and ensure timely reporting of patient status to the interdisciplinary team
  • Assist with discharge planning and develop follow-up recommendations for care continuity 4. Collaboration and Communication
  • Coordinate care with physicians, nurses, and other healthcare professionals
  • Attend case conferences or team meetings to discuss patient progress and goals
  • Communicate effectively with scheduling staff to optimize visit frequency and patient satisfaction 5. Safety and Quality Assurance
  • Adhere to infection control and patient safety guidelines within the home environment
  • Conduct regular risk assessments of the home to identify and mitigate hazards
  • Engage in ongoing professional development, staying current with best practices in home health Requirements 1. Licensure and Certification
  • Valid and active state PT license
  • CPR and first aid certification (often required)
  • Valid driver's license and reliable transportation 2. Education and Experience
  • Doctorate in Physical Therapy (DPT) preferred, Master's may be accepted depending on state regulations
  • Minimum of 1-2 years of experience in a clinical setting, with home health experience preferred
  • Knowledge of home health standards and compliance (e.g., Medicare, HIPAA) 3. Technical Skills
  • Proficient in conducting assessments, developing care plans, and documentation using electronic health record (EHR) systems
  • Ability to educate patients and families on self-care, prevention, and management
  • Familiarity with therapeutic modalities and equipment used in home health settings 4. Soft Skills
  • Strong communication and interpersonal skills to connect with diverse patients and families
  • Excellent organizational and time-management skills, as home health requires independent scheduling
  • Problem-solving skills, particularly for adapting care in home environments 5. Physical and Availability Requirements
  • Ability to lift and assist patients as needed, and to conduct therapy in various home environments
  • Flexibility to accommodate patient scheduling needs, including possible weekends or holidays
  • Comfort with travel and a willingness to serve patients across a designated area
