As a Home Health Physical Therapist (PT) , you'll have the opportunity to make a meaningful impact by helping patients regain mobility, strength, and independence—all within the comfort of their own homes. You'll work closely with our skilled nursing and therapy teams to assess, plan, and deliver evidence-based care that promotes safety, recovery, and improved quality of life. Key Responsibilities Conduct comprehensive physical therapy evaluations in patients' homes to assess strength, balance, range of motion, pain, and functional mobility. Develop and implement individualized treatment plans focused on achieving patient-centered goals, improving function, and promoting independence. Provide therapeutic exercises, gait training, transfer training, and balance activities tailored to each patient's needs and abilities. Educate patients and caregivers on home exercise programs , fall prevention, safety techniques, and adaptive strategies for daily activities. Monitor patient progress and update care plans as needed, ensuring continuity and quality of care. Collaborate with the interdisciplinary home health team —including RNs, OTs, speech therapists, and medical social workers—to coordinate and optimize patient outcomes. Accurately document all visits, interventions, and progress notes in the electronic medical record (EMR) system per agency and regulatory guidelines. Participate in case conferences and care coordination to support holistic, team-based patient care. Provide compassionate, professional service that aligns with Kiwi Home Health's mission of excellence in home-based care. Qualifications Current California Physical Therapist (PT) License required. Minimum 1 year of home health experience preferred. Kinnser/Wellsky EMR experience highly preferred. Current CPR certification . Reliable transportation and valid driver's license required for travel to patient homes throughout San Diego County. Strong communication, documentation, and time management skills. Compassionate, patient-centered approach with the ability to work independently while collaborating effectively with the care team.
