Full Time Position Overview We are seeking a highly skilled and compassionate Physical Therapist (PT) to join our rehabilitation team on a Full time basis. In this role, you will be responsible for evaluating and treating residents in a Skilled Nursing Facility (SNF) to help them regain functional independence and improve their quality of life. This position offers a competitive hourly rate. Key Responsibilities •

Evaluations & Assessments:

Conduct comprehensive initial evaluations of residents to determine their physical condition, functional limitations, and rehabilitation potential. •

Plan of Care:

Develop and implement individualized, evidence-based treatment plans focused on gait training, strength building, and pain management. •

Supervision:

Provide clinical supervision to Physical Therapist Assistants (PTAs) and Rehab Aides to ensure the highest standards of care. •

Interdisciplinary Collaboration:

Participate in care conferences and work closely with nursing, occupational therapy, and speech-language pathology teams to coordinate discharge planning. •

Documentation:

Maintain timely and accurate clinical documentation (evaluations, progress notes, and discharge summaries) in compliance with Medicare and state regulations. •

Patient Advocacy:

Educate residents and their families on safety, mobility techniques, and home exercise programs. Qualifications •

Education:

Graduate of an accredited Physical Therapy program (DPT preferred). •

Licensure:

Current, valid California Physical Therapist License in good standing. •

Certification:

Basic Life Support (BLS) certification required. •

Experience:

Previous experience in a Skilled Nursing Facility (SNF) or geriatric care setting is highly preferred. •

Knowledge:

Proficiency with EMR systems and a strong understanding of PDPM and long-term care regulations. Physical Requirements • Ability to assist with patient transfers and lift/move up to 50 lbs.

Pay:

$55.00 - $62.00 per hour
