Role:
As a Registered Physical Therapist, you will play a critical role in helping patients recover from injuries, manage chronic conditions, and improve their overall physical function and mobility in their homes. You will assess patients' physical abilities, develop personalized treatment plans, and implement therapeutic interventions to promote healing and prevent further injury. Your expertise will guide patients through exercises and activities designed to restore strength, flexibility, and coordination, while also educating them on injury prevention and wellness strategies. Collaboration with physicians, nurses, and other healthcare professionals will be essential to ensure comprehensive patient care. Ultimately, your work will significantly enhance patients' quality of life by enabling them to regain independence and return to their daily activities safely and effectively.
Minimum Qualifications:
Valid and current license to practice as a Physical Therapist in California. Graduation from an accredited physical therapy program or equivalent. Strong knowledge of anatomy, physiology, kinesiology, and therapeutic modalities. Demonstrated ability to perform patient assessments and develop effective treatment plans. Excellent communication and interpersonal skills to interact effectively with patients and healthcare teams.
Preferred Qualifications:
Experience working in Home Health or outpatient settings. Certification in specialized areas such as orthopedics, geriatrics, or sports physical therapy. Proficiency with electronic medical records (EMR) systems and documentation software. Additional training in manual therapy techniques or neuromuscular re-education. Bilingual abilities to serve diverse patient populations.
Responsibilities:
Conduct thorough patient evaluations to assess physical condition, functional limitations, and rehabilitation needs. Develop and implement individualized treatment plans based on patient assessments and clinical guidelines. Administer therapeutic exercises, manual therapy, and other interventions to improve patient mobility and reduce pain. Monitor patient progress and adjust treatment plans accordingly to achieve optimal outcomes. Educate patients and their families on home exercise programs, injury prevention, and health maintenance. Collaborate with multidisciplinary healthcare teams to coordinate patient care and ensure continuity. Maintain accurate and detailed patient records in compliance with legal and ethical standards. Stay current with advancements in physical therapy techniques and best practices through continuing education.
