Job Description

Physical Therapist (PT) Marin Post Acute | Part-Time & Per Diem $65-$70/hour Empower Recovery. Restore Independence. Change Lives. At Marin Post Acute, we are committed to delivering exceptional rehabilitation services that help residents regain strength, mobility, and confidence. We are currently seeking compassionate and motivated Physical Therapists (PTs) to join our therapy team on a Part-Time or Per Diem basis. If you're passionate about patient-centered care and enjoy helping individuals achieve meaningful recovery goals, we'd love to hear from you. What You'll Do Evaluate residents and develop individualized treatment plans Provide skilled physical therapy services to post-acute, rehabilitation, and long-term care residents Collaborate with physicians, nursing staff, therapy professionals, and families to ensure optimal outcomes Monitor progress and adjust treatment plans based on resident needs Educate residents and caregivers on mobility, safety, strengthening, and functional independence Maintain accurate and timely documentation in compliance with regulatory and reimbursement requirements Participate in interdisciplinary care planning and discharge planning activities
Qualifications Required:
Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program Current California Physical Therapist license in good standing Strong clinical assessment and treatment skills Excellent communication and interpersonal abilities
Preferred:
Skilled Nursing Facility, Post-Acute, Rehabilitation, or Long-Term Care experience Familiarity with Medicare documentation and reimbursement guidelines Experience treating orthopedic, neurological, cardiac, and medically complex patients Why Marin Post Acute?
