Minimum 1 year About the Role We are hiring a Physical Therapist for a 13-week contract in Napa, CA. This is a home health role covering assigned territories, including Vacaville, Fairfield, Rio Vista, Suisun City, Dixon, Vallejo, Benicia, Napa, American Canyon, and Yountville. Responsibilities Provide physical therapy services in a home health setting Complete evaluations, routine visits, reassessments, recertifications, and discharges Develop and follow individualized patient treatment plans Support mobility, strength, balance, safety, and functional independence Travel within assigned territory and outside the area as needed Complete patient documentation accurately and timely Communicate with patients, physicians, and care team members Meet weekly productivity and visit expectations Requirements Active California Physical Therapist license required Minimum 1 year of Physical Therapy experience required Home health experience preferred

BLS/BCLS

certification required Valid driver's license, auto insurance, and auto registration required Must be willing to travel within assigned territory Weekend coverage may be required Must be willing to see COVID+ and

COVID PUI
