Schedule/Hours:

Weekend coverage may be required Coverage primarily in San Jose, Santa Clara, and adjacent cities Occasional travel outside designated area to secure a full caseload

Responsibilities:

Assess, plan, and implement physical therapy services across client homes Complete and document OASIS assessments, recertifications, evaluations, follow-ups, and discharges Collaborate closely with physicians, patients, and care teams for optimal patient outcomes Maintain productivity expectations and comprehensive records, including required point system Willingness to see Covid+ and Covid PUI patients as needed

Qualifications:

Current California Physical Therapy License Basic Life Support (BLS) certification Valid driver's license, auto insurance, and registration Up-to-date vaccinations: TB (annual), MMR, Varicella, HepB, TDap (within 10 years), Flu (annual) Drug screening and physical exam clearance Flexibility to travel across designated territories and work weekends as needed
