We are a 59-bed skilled nursing and rehabilitation center in Santa Maria, California. PRN also available. Valley Oaks is an exceptional skilled nursing facility, offering a high level of patient care. Our range of services includes short-term and long-term options to suit any individual's needs - perfect for hospitals, medical centers or individuals in need! We're excited about the opportunity to share our story with the community and make sure everyone can benefit from quality healthcare.
Essential Duties and Responsibilities:
Plan treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing Assure all treatment is delivered in accordance with an established plan of care Provide clinical support and instruct patients, families, and caregivers Monitor patient response to treatment intervention Complete required forms and documentation in accordance with company policy and state/federal regulations Consulting with patients to learn about their physical condition. Assessing and interpreting patient evaluations and test results. Developing treatment plans using a variety of treatment techniques. Creating personalized fitness-oriented health care programs for patients. Administering medically prescribed physical therapy treatments to relieve pain and improve mobility. Advising patients on exercise techniques. Advising patients and their families about in-home treatment options. Providing educational information about injury prevention, ergonomics and ways to promote physical health. Consulting and collaborating with other healthcare professionals. Documenting patient care history. Complying with rules, regulations, and procedures.
