Minimum Requirements Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Active California Physical Therapist license required to start the assignment. Current BLS certification required from an approved vendor per facility policy. HomeCare HomeBase experience required. EPIC electronic medical record experience required. Valid driver's license, current automobile insurance, and reliable automobile required. Must be at least 21 years of age. Prior home health or acute care physical therapy experience preferred. CPR certification or Basic Life Support (BLS or equivalent) is required.

Assignment Details Facility Type:

Hospital (home health services managed through hospital-affiliated program).

Shift:

Days.

Shift Hours:

Monday-Friday, 0800-1700.

Weekly Hours:

40 hours per week.

Assignment Duration:

16-week travel assignment.

Clinical Role:

Manage a home health patient caseload independently and collaborate with interdisciplinary team members to meet care goals.

Travel Requirements:
