Employee Referral Program:
Full-time Physical Therapists who refer another full-time PT are eligible for a $3,000 referral bonus, and the referred PT also receives a $3,000 bonus after eligibility requirements are met Sign-on bonus paid in two increments, one at a year and the second and two years. Territory This position serves patients throughout the Stockton market, generally ranging from Lodi to Stockton and surrounding communities. Patient schedules are designed to be geographically efficient, and clinicians are not usually expected to cover the entire territory in a single day. Essential Responsibilities Provide skilled physical therapy services in accordance with physician orders and the established plan of care Evaluate patient functional status, mobility, strength, gait, range of motion, and rehabilitation potential Develop and implement individualized treatment plans Educate patients and caregivers regarding treatment goals, therapeutic exercises, safety, and use of adaptive equipment Communicate patient progress, treatment response, and changes in condition to physicians and the interdisciplinary team Participate in care planning, case conferences, and discharge planning activities Complete clinical documentation and progress summaries in accordance with company policy Identify the need for additional home health services and coordinate appropriate referrals Supervise Physical Therapy Assistants in accordance with state regulations and company policy Provide consultation and education to caregivers and other healthcare team members as appropriate Qualifications Graduate of an accredited Bachelor's, Master's, or Doctor of Physical Therapy program Current California Physical Therapy license in good standing Minimum of two years of Physical Therapy experience preferred Home Health experience preferred but not required Current CPR certification Excellent communication, organizational, and clinical skills Valid driver's license, reliable transportation, and current automobile insurance Benefits Full-time employees are eligible for: Blue Cross Blue Shield or Kaiser Medical Insurance Dental and Vision Insurance 401(k) with Company Match Life Insurance and Supplemental Insurance Options 7 Paid Holidays Annually Accrued Time Off (ATO) begins accruing upon hire and becomes available after 90 days 10 ATO Days during the first year of employment, with increased accrual based on tenure $15,000 Sign-On Bonus Available, paid in two increments (One at first anniversary, second at second anniversary of hire) $3,000 Employee Referral Bonus for both the referring PT and newly hired PT after eligibility requirements are met Pre-Employment Requirements Current Tuberculosis (TB) Test or Chest X-ray Titers or proof of immunity for Varicella, MMR, and Hepatitis B Proof of Tdap vaccination Proof of annual Influenza vaccination Current physical examination Join a team that values clinical excellence, supports therapist success, and provides the tools and flexibility needed to thrive in home health.
