Overview Physical Therapist - Home Health in Stockton, California.
Start date:

Duration:
13 weeks. Guaranteed 40 hours per week.
Shift:
Days. This travel assignment takes place in an outpatient rehabilitation setting with a home health focus, offering opportunities within a private practice recognized for excellence in the field.

2,110 to 2,241 per week Guaranteed hours: 40 per week Contract duration: 13 weeks
Shift:
Days Start date: 07/01/2026 Work setting: outpatient rehabilitation environment Responsibilities Evaluate and treat patients to improve mobility, function, and strength; implement individualized rehabilitation programs Develop and modify treatment plans, monitor progress, and adjust interventions as needed Document evaluations, treatment notes, progress reports, and discharge summaries in the electronic medical record Supervise PT Assistants and Aides, coordinating care across therapy sessions Communicate with physicians and other members of the care team to ensure high-quality patient care Maintain HIPAA privacy standards; participate in privacy training and protect patient information Assist with scheduling, referrals, and follow-up activities; collaborate with front office staff as needed Demonstrate proficiency in DX and ICD-10 coding for proper billing of services Meet physical demands of the role, including standing/walking for long periods, lifting up to 50 pounds, and assisting with patient transfers Requirements Doctor of Physical Therapy (DPT) degree Active Physical Therapist license in California or eligibility to obtain licensure with plan to test in the near term Relevant experience in physical therapy rehabilitation settings; home health or outpatient experience preferred Proficient with electronic medical records, documentation, and ICD-10 coding; strong adherence to privacy and compliance standards Ability to work effectively in an outpatient rehabilitation environment Continuing education courses encouraged to support ongoing professional development Benefits and Support Dedicated recruiter and assignment support from Healthforce Healthcare LLC Credentialing and onboarding assistance to facilitate a smooth start Access to a broad network of travel assignments and ongoing assignment support Opportunities for contract extension based on performance and organizational needs Find more opportunities Find more opportunities at
