  • 170 per Visit Full-Time / Part-Time / Per Diem Job Overview United Home Health is seeking a dedicated and compassionate Physical Therapist (PT) to join our growing home health team.
The Physical Therapist will provide skilled therapy services to patients in their homes, helping them improve mobility, strength, balance, safety, and overall independence. Responsibilities Perform comprehensive physical therapy evaluations and reassessments Develop individualized treatment plans based on patient needs and goals Provide therapeutic exercises, gait training, balance training, and fall prevention education Evaluate and recommend assistive devices and home safety modifications Educate patients and caregivers on treatment plans and home exercise programs Collaborate with physicians, nurses, and other healthcare professionals Complete accurate and timely documentation in accordance with agency and regulatory requirements Monitor patient progress and adjust treatment plans as needed Qualifications Active California Physical Therapist (PT) license Graduate of an accredited Physical Therapy program Home health experience preferred, but not required Current CPR/BLS certification Valid driver's license, auto insurance, and reliable transportation Strong clinical assessment, communication, and documentation skills Ability to work independently and manage a flexible schedule Benefits Competitive pay: $150
