Category Therapy - Hospital - PT Job Type Travel A healthcare facility in Tracy, CA is seeking a licensed Physical Therapist to provide comprehensive therapy services in an outpatient setting. This travel opportunity allows for flexible relocation and offers the chance to advance patient mobility and independence through skilled evaluation and targeted treatment plans.
Key Responsibilities:
Conduct thorough evaluations of assigned patients to develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician directives. Oversee and guide discipline-specific therapy services, ensuring functional treatment programs support effective return to community activities. Educate patients, families, students, and colleagues on therapy processes and best practices. Maintain professional standards, uphold confidentiality, and follow organizational Code of Conduct. Promote patient and employee safety through adherence to established safety protocols and active hazard identification. Complete timely and accurate documentation including evaluations, care plans, quality assessments, and billing in compliance with regulatory standards. Participate in student mentorship, fostering a positive learning environment. Engage in continuous professional development to enhance clinical skills and align with organizational goals. Comply with State Practice Acts and regulatory requirements, maintaining active licensure and ethical practice.Desired Qualifications:
Valid Physical Therapy licensure in accordance with state regulations. Strong clinical reasoning and patient-centered care skills. Experience in outpatient physical therapy preferred. Ability to document thoroughly and comply with insurance and regulatory guidelines. Commitment to patient and staff safety principles. Enthusiasm for teaching and professional growth.About Tracy, CA:
