Strategic Treatment:
Perform evaluations and develop evidence-based plans that drive real resident outcomes. •
Team Leadership:
Provide clinical supervision to PTAs and mentor Restorative Aides to ensure continuity of care. •
Collaborative Care:
Engage in patient care conferences and utilization reviews alongside a dedicated interdisciplinary team. •
Smooth Transitions:
Facilitate successful discharges by recommending DME (walkers, braces, etc.) and empowering families through caregiver training. •
Compliance:
Maintain meticulous documentation in alignment with the PT Board and payer requirements. What You Bring to the Team •
Licensed:
A valid, current California Physical Therapist (PT) License. •
Independent:
Strong clinical judgment and the ability to thrive in an autonomous environment. •
Interpersonal:
Exceptional communication skills to build rapport with residents and families. •
