Hours Guaranteed Weekly Pay Rate:
$65•68 per hour Join a dedicated rehabilitation team in a well-established Skilled Nursing Facility in Yountville, California. We are seeking a compassionate and motivated Physical Therapist who is passionate about helping patients regain mobility, improve functional independence, and enhance their quality of life.
Responsibilities:
Evaluate and assess patients' physical therapy needs. Develop individualized treatment plans based on patient goals and clinical findings. Provide skilled therapeutic interventions to improve strength, balance, mobility, and function. Monitor patient progress and modify treatment plans as needed. Collaborate with physicians, nursing staff, and interdisciplinary team members. Maintain accurate and timely documentation in accordance with facility and regulatory requirements. Educate patients and caregivers on treatment plans and home exercise programs.
Qualifications:
Active California Physical Therapist (PT) License. Graduate of an accredited Physical Therapy program. SNF or geriatric rehabilitation experience preferred. Strong communication and interpersonal skills. Ability to work independently and as part of a collaborative team.
