We are hiring a Full-Time Physical Therapist (PT) for a highly flexible Home Health position. This role offers one-on-one patient care, manageable territories, and strong earning potential—along with the ability to build your own schedule and work around your life.
Your Role:
As a Physical Therapist, you will evaluate, plan, and provide individualized treatment for patients in their homes while collaborating with a supportive clinical team.
What You'll Do:
Provide high-quality patient care using approved therapy techniques
Evaluate patients and establish clear, measurable goals
Develop and implement personalized treatment plans
Supervise and collaborate with PTAs and HHAs
Document patient progress accurately and on time
Educate patients and families on exercises, mobility, and equipment use
Assess home environments and recommend safety modifications
Coordinate care with nursing, social work, and other clinical staff
Maintain HIPAA compliance and patient confidentiality

Requirements:
Bachelor's, Master's, or Doctorate degree in Physical Therapy
Active Physical Therapy license in the state
Valid driver's license and reliable transportation
