In this vital role, you will assess, develop, and implement personalized treatment plans to help patients regain mobility, reduce pain, and improve overall quality of life. Your expertise will support a diverse patient population across inpatient hospital settings, outpatient clinics, home health environments, and community care programs. This position offers an exciting opportunity to make a meaningful impact by guiding patients through their recovery journey with compassion and professionalism. Duties Conduct comprehensive patient evaluations to determine functional limitations and develop tailored care plans Implement evidence-based physiotherapy techniques, including trigger point therapy, orthopedics, geriatrics, pediatrics, and sports rehabilitation Assist patients in improving mobility, strength, flexibility, and pain management through targeted interventions Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless patient care transitions Educate patients and their families on injury prevention, home exercises, and community resources for ongoing recovery Maintain accurate documentation of patient progress using electronic medical record (EMR) systems Participate in ongoing professional development to stay current with physiotherapy advancements and best practices Requirements Valid licensure or registration as a licensed Physical Therapist in the relevant jurisdiction Proven experience in inpatient hospital care, outpatient clinics, or home health environments Strong knowledge of anatomy, physiology, and medical terminology relevant to physiotherapy practice Experience with discharge planning, care plans, and working within EMR systems Familiarity with acute care settings and hospital workflows Ability to apply physiotherapy techniques such as trigger point therapy and orthopedics effectively Excellent communication skills for patient education and interdisciplinary collaboration Knowledge of pediatric and geriatric care principles is highly desirable Prior experience working with home & community care programs is a plus Join us to help transform lives through expert physiotherapy services!
