Physical Therapist Part Time Choice Health At Home - 3.6 Englewood, CO Job Details Part-time 10 hours ago Qualifications CPR Certification BLS Certification Driver's License Physical Therapy License Full Job Description As a Home Health Physical Therapist, you'll practice at the top of your license—bringing skilled, one-on-one care directly into patients' homes where it matters most. You'll independently evaluate patients, design personalized, evidence-based treatment plans, and help restore mobility, reduce pain, and improve daily function and independence. You'll also lead and support Physical Therapist Assistants (PTAs), ensuring that care is delivered safely, effectively, and aligned with each patient's goals. Working closely with an interdisciplinary team, you'll continuously adjust treatment plans to drive the best possible outcomes. In this role, you will: Conduct comprehensive evaluations and establish clinical diagnoses to guide care Develop and implement individualized treatment programs focused on real progress and function Reassess patients and modify care plans to maximize recovery and results Apply infection control and safety standards in all patient interactions Maintain accurate, timely, and compliant documentation Communicate and collaborate with patients, families, and care teams Supervise PTAs to ensure quality and continuity of care This role offers autonomy, flexibility, and the opportunity to truly impact lives—helping patients regain independence and improve their quality of life in the comfort of their own homes. Requirements Successful completion of accredited physical therapy program and state licensure exam. Valid Physical Therapy license 1 year of home health experience preferred CPR certification Valid Driver's license Valid Auto Insurance HomeCare HomeBase experience preferred Physical Demands Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods. Must be able to lift, carry, pull, or push up to 100 lbs..
