Duties Assess patients' physical conditions by conducting thorough evaluations of mobility, strength, and functional abilities using medical terminology and clinical expertise. Develop and implement individualized care plans that incorporate evidence-based physiotherapy practices, including trigger point therapy, orthopedics, geriatrics, pediatrics, and community care strategies. Administer therapeutic interventions such as manual therapy, exercise programs, and modalities tailored to each patient's needs. Monitor patient progress regularly, adjusting treatment plans to optimize outcomes while ensuring safety and comfort. Coordinate discharge planning by collaborating with multidisciplinary teams to ensure seamless transitions from hospital to home or community settings. Maintain accurate documentation within electronic medical records (EMR) systems, including detailed notes on treatment sessions, patient responses, and care objectives. Educate patients and their families on injury prevention, physiotherapy exercises, and home care routines to promote long-term wellness. Qualifications Valid licensure as a Physical Therapist (PT) with current registration in the relevant jurisdiction. Demonstrated experience in inpatient hospital settings, outpatient clinics, or home health environments with familiarity in acute care and hospital workflows. Strong knowledge of physiology, anatomy, medical terminology, and orthopedic principles essential for effective patient assessment and treatment. Proven ability to develop comprehensive care plans that incorporate discharge planning and community-based care strategies. Experience working with diverse populations including pediatrics, geriatrics, and individuals requiring specialized therapies such as trigger point therapy or orthopedics. Proficiency with EMR systems for documentation and patient management. Excellent communication skills to collaborate with healthcare teams and educate patients effectively. Additional skills in home & community care experience and outpatient physiotherapy are highly desirable for delivering holistic patient-centered services. Pay:
$42.00 - $55.00 per hour
