Job Description

Overview Physical Therapist - Home Health in Salida, Colorado. This travel, temporary assignment begins on 10/08/2026 and spans 13 weeks in a rural, critical access setting. The position offers a weekly pay range of $2,157-$2,308 with guaranteed 36 hours per week. Shift is four 9-hour days per week (7:00am-5:00pm) Monday through Thursday, with Fridays and weekends off. Work will be in a rural healthcare clinic environment within a critical access context, with experience in outpatient and orthopedic care required.

Setting:
Rural healthcare clinic in a critical access environment; outpatient/orthopedic focus Responsibilities Perform thorough patient examinations to determine impairments, functional limitations, and disabilities; establish diagnosis, prognosis, and appropriate intervention strategies within the PT scope of practice. Design, implement, and adjust individualized therapeutic plans aimed at reducing impairments and restoring functional independence, with emphasis on outpatient and orthopedic care. Deliver therapeutic interventions including therapeutic exercise, gait training, manual therapy, modalities, and patient education to promote self-management and injury prevention. Maintain accurate patient records (orders, diagnoses, treatment plans, progress notes, attendance, discharge summaries) and ensure timely submission of charges for services rendered. Collaborate with the care team to coordinate care, educate patients and families, and contribute to evidence-based practice within a rural setting. Requirements Active Colorado Physical Therapist licensure (or permit while awaiting licensure) Graduation from an
AMA-APTA
