Job Description

Physical Therapist - Geriatric Care Program Thornton, CO Full-Time | Monday-Friday | 8:00 AM - 4:30 PM 14-Week Contract We are seeking a compassionate and experienced Physical Therapist to join a collaborative healthcare team serving adults ages 55 and older in a community-based care program. This role focuses on helping participants maintain independence, mobility, and quality of life through individualized rehabilitation and therapy services. Responsibilities Evaluate participants for acute, restorative, and maintenance rehabilitation needs Develop and implement individualized physical therapy treatment plans Integrate therapy services into participant Plan of Care (POC) based on physician orders Provide therapeutic exercises, mobility training, positioning, and modality treatments Educate participants and caregivers on exercises, mobility, safety, and use of durable medical equipment (DME) Collaborate closely with interdisciplinary team members including nursing, providers, occupational therapy, speech therapy, and caregivers Conduct home visits and educational sessions as needed Delegate treatment plans appropriately to Physical Therapist Assistants and Rehab Aides Monitor participant progress and document all services in a timely and accurate manner Assist with ordering, inventory, maintenance, and documentation of DME Participate in interdisciplinary meetings, quality improvement activities, and peer reviews Provide guidance and training to staff regarding participant mobility and rehabilitation needs
Qualifications:
Required Bachelor's Degree in Physical Therapy Graduation from an accredited Physical Therapy program Active Physical Therapist license in the state of Colorado Minimum of 1 year of experience working with frail or elderly populations Strong communication and collaboration skills Preferred 2+ years of Physical Therapy experience MSPT or DPT preferred Bilingual skills are a plus What We Offer Consistent Monday-Friday schedule with no weekends Collaborative interdisciplinary team environment Opportunity to make a meaningful impact in the lives of older adults Supportive and mission-driven healthcare team Competitive compensation package Apply today to join a team dedicated to helping older adults live safely and independently in their communities. #
