Full Job Description Home Health Physical Therapist PT Job Summary:
PathWell Health, a trusted and community-focused home health care provider, is seeking a highly skilled and compassionate Physical Therapist to join our team. As a Home Health Physical Therapist, you will work with patients in their homes to improve mobility, relieve pain, increase strength, and promote overall well-being. Essential Duties and Responsibilities:
Develop, implement, and adjust a conditioning/rehabilitation plan of care in partnership with patients, representatives, caregivers, and physicians Test and screen patients' physical strength to assist physicians in evaluating their level of function Educate and instruct patients, family members, or representatives about exercise programs and therapeutic procedures Accurately document observations, interventions, and evaluations pertains to patient care management and services provided Provide direct patient care as prescribed, including evaluation and treatment planning Review physicians' referrals and patients' medical records to determine diagnosis and physical therapy treatment required Minimum Qualifications (Knowledge, Skills, and Abilities): Graduation from an approved Physical Therapy curriculum Current and valid Connecticut Physical Therapist license Recent work experience in home health (preferred) Driver's License, Car Insurance, and Car Registration Strong clinical assessment, organizational, and communication skills Ability to handle multiple assignments simultaneously Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing, and documentation Bilingual (Spanish) preferred Strong interpersonal skills and good judgment Ability to work well under pressure and stay calm and focused in emergent situations Be highly self-motivated and results-driven A desire for further education to keep up with the ever-changing medical advances

The work environment is typically a patient's home, with occasional visits to medical facilities or our office.
Salary:
$90,000 - $120,000
