- Evaluate each patient's functional status and determine Physical Therapy needs.
- Develop and implement a plan of care in conjunction with the MD and in collaboration with the interdisciplinary team.
- Implement a plan of care to provide safe, quality care to multiple patients on a daily basis.
- Perform patient reassessments and discharges at appropriate times.
- Be responsible for completing daily clinical documentation.
- Provide supervision to/work collaboratively with PTAs to provide exceptional care to achieve patient goals.
- Organize and prioritize caseload needs on a daily basis to ensure patient care needs are met.
Qualifications:
- Degree from an accredited Physical Therapy Program
- Valid CT Physical Therapy License
- Valid CT Driver's License
- Minimum of 1 year of homecare experience preferred but open to mentoring those without homecare experience
- Strong organizational and communication skills
