  • Full-time Overview Join our dynamic healthcare team as a Physical Therapist specializing in Home Care, where you will play a vital role in restoring and enhancing patients' mobility and independence within their own homes.
This position offers an exciting opportunity to deliver personalized, compassionate care across diverse patient populations, including pediatrics, geriatrics, and individuals recovering from acute or chronic conditions. As a key member of our home health services, you will utilize your expertise to develop tailored care plans, perform comprehensive assessments, and promote optimal recovery in the comfort of patients' environments. Your dedication will directly impact quality of life and foster positive health outcomes. Duties Conduct thorough evaluations of patients' physical conditions, including mobility, strength, and functional abilities, utilizing your physiology and anatomy knowledge. Develop and implement individualized care plans that incorporate evidence-based physiotherapy techniques such as trigger point therapy and orthopedics rehabilitation. Deliver hands-on therapy sessions focusing on improving patient outcomes through therapeutic exercises, manual therapy, and patient education. Coordinate discharge planning with multidisciplinary teams to ensure seamless transition from inpatient or outpatient settings to home care. Monitor patient progress regularly, adjusting treatment strategies as needed to meet evolving health goals. Maintain accurate documentation of treatments, progress notes, and care plans within electronic medical record (EMR) systems while adhering to healthcare regulations. Educate patients and caregivers on injury prevention, activity modifications, and self-management strategies to support ongoing recovery at home. Skills Strong understanding of medical terminology, physiology, and human anatomy essential for accurate assessment and treatment planning. Proven experience in inpatient hospital settings, outpatient clinics, or home health environments with a focus on patient care continuity. Expertise in discharge planning and managing care transitions across various healthcare settings. Familiarity with EMR systems for efficient documentation and communication of patient information. Knowledge of pediatric, geriatric, orthopedics, geriatrics, and community care practices to address diverse patient needs effectively. Ability to apply physiotherapy techniques such as trigger point therapy within treatment protocols. Excellent communication skills for collaborating with healthcare teams and educating patients and families about their care plans.
